Connect 4 Health - Readiness Questionnaire

 

Starting school for the first time is a significant milestone for every child, and often a scary and exciting time for them and their parents/carers. Connect for Health, your School Nursing service in Warwickshire, want to make sure all children and families have the support they need for their health, wellbeing and development to prepare for this.

 

That’s why they’re asking parents to complete the School Readiness Health Questionnaire. This questionnaire helps to identify if children have any health and wellbeing needs that they may require support with so they can reach their full potential as they enter education.

 

All your answers are treated confidentially and stored securely online. Based on your answers, parents will be offered advice and guidance, as well as the opportunity to contact the team for further support. The questionnaire should be completed online by all parents/carers before their child starts school in the September.

 

You can fill in the questionnaire online using a phone, tablet, laptop or computer, and it takes approximately 5-10 minutes to complete. To start your questionnaire and find out more information, follow the link below!
